Cost Protection Plan Claim Process

For students/parents have paid for the Cost Replacement Plan on mobile devices, the following process must be followed:

  • Each student/parent must complete a Claim Form--Notice of Damage.
  • The Claim Form is submitted to the VHS Library staff --completely filled out. 
  • The site reviews and verifies there was coverage on the device. 
  • If MDCPP does not apply, the appropriate charge will be added onto the student's account.
